WAIVER ASSUMPTION OF RISK AND AGREEMENT TO HOLD HARMLESS By signing this form you aggree to the following terms. I understand that putting my dog in daycare is not without risk. Some of the dogs that I(we) will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest of care. I hereby waive and release Sandy Braunwarth, Wildsands, all assistants, or agents from any and all liability of any nature, for injury or damage which I or my dog may suffer, including specifically , but not without limitation, any injury or damage resulting from the action of any dog, and I expressly assume the risk of any such damage or injury while my dog is being boarded or in daycare or while on the training / daycare grounds or the surrounding area thereto. In consideration of and as inducemet to the acceptance of my application for daycare, I hereby agree to indemnify and hold harmless this facility, owner, trainer, assistants and any agents from any and all claims, or claims by any member of my family or any other person accompanying me to any daycare session or function of the facility or while on the grounds or the surrounding area thereto as a result of any action by any dog, including my own. Vaccination Policy: Vaccinations must be up to date . A copy of your dogs last wellness exam and vaccination history and parasite prevention is required at drop off.
By signing this form the owner(s) agree to the terms and have read the waiver listed. Wildsands Petcare has the right to refuse any pet. A signature may also be required at time of drop off.
